Cholesterol results are one of the most commonly misunderstood blood tests in primary care. Many people focus entirely on their "total cholesterol" number and either panic or breathe a sigh of relief based on that alone. The reality is more nuanced โ and in some ways more reassuring. Let me walk you through what each component actually means.
What Is Cholesterol and Why Do We Measure It?
Cholesterol is a fatty substance that your body needs to build cells, produce hormones, and make vitamin D. It's not inherently dangerous โ in fact, your liver makes most of it naturally. The problem arises when certain types of cholesterol accumulate in your arteries, contributing to the buildup of fatty plaques (atherosclerosis) that can eventually lead to heart attack or stroke.
Your cholesterol blood test โ usually called a "lipid profile" or "fasting lipids" โ measures four components: total cholesterol, HDL (high-density lipoprotein), LDL (low-density lipoprotein), and triglycerides. Each tells a different story.
The Four Numbers on Your Cholesterol Report
Total Cholesterol
This is the headline figure โ the sum of all cholesterol in your blood. However, it's the least useful number in isolation. A total cholesterol of 6.0 mmol/L could be perfectly fine if most of it is "good" HDL, or it could be concerning if it's mostly "bad" LDL. Context is everything.
HDL โ High-Density Lipoprotein ("Good" Cholesterol)
HDL is often called "good" cholesterol because it acts as a clean-up crew โ it picks up excess cholesterol from your bloodstream and artery walls and transports it back to the liver for processing. Higher HDL is better. An HDL below 1.0 mmol/L in men or below 1.2 mmol/L in women is considered a cardiovascular risk factor in its own right.
LDL โ Low-Density Lipoprotein ("Bad" Cholesterol)
LDL is the main carrier of cholesterol to your cells โ but when there's too much of it, it can deposit in artery walls and contribute to plaque. Lower LDL is generally better, particularly for people who already have heart disease or diabetes. For most healthy adults without other risk factors, an LDL below 3.0 mmol/L is the general target.
Triglycerides
Triglycerides are a type of fat stored in your blood, derived largely from the food you eat โ particularly refined carbohydrates, sugar, and alcohol. Elevated triglycerides (above 1.7 mmol/L fasting) can raise cardiovascular risk, particularly in combination with low HDL. Very high triglycerides (above 10 mmol/L) can cause pancreatitis and are a medical urgency.

Your total cholesterol to HDL ratio is the calculation many clinicians find most useful. Divide your total cholesterol by your HDL. A ratio below 4.0 is desirable; above 6.0 significantly raises cardiovascular risk. Ask your GP for this calculation if it's not on your results letter.
Your QRISK Score: The Bigger Picture
Cholesterol numbers alone don't determine whether you need treatment. Your GP will use a tool called QRISK3 to calculate your 10-year risk of a cardiovascular event (heart attack or stroke). This algorithm combines your cholesterol results with age, sex, blood pressure, smoking status, family history, and presence of conditions like type 2 diabetes or chronic kidney disease.
In the NHS, a QRISK3 score of 10% or above (meaning a 10% or higher chance of a heart attack or stroke in the next 10 years) is generally the threshold at which statin therapy is discussed. However, treatment decisions are always individualised โ some people with lower scores may still benefit, and some with higher scores may prefer lifestyle-only management.
What Raises LDL and Triglycerides?
Understanding what drives your cholesterol up helps you understand how to bring it down. The main dietary drivers of raised LDL are saturated fats (found in fatty red meat, full-fat dairy, butter, and tropical oils like palm and coconut oil) and trans fats (largely eliminated from UK processed food but still present in some imported products).
Triglycerides are most responsive to dietary carbohydrates and sugar โ particularly refined carbs, white bread, pastries, sweetened drinks, and alcohol. Reducing these can produce rapid and significant drops in triglyceride levels, often within weeks.
Beyond diet, genetics play a significant role. Familial hypercholesterolaemia (FH) is an inherited condition affecting approximately 1 in 250 people in the UK, causing very high LDL regardless of diet. If your total cholesterol is above 7.5 mmol/L, or your LDL above 4.9 mmol/L, and you have a family history of early heart disease, your GP may refer you for FH assessment.
What About Statins?
Statins are among the most studied and effective medications in modern medicine. They reduce the liver's production of cholesterol and have been shown to reduce cardiovascular events in people at moderate and high risk, even when cholesterol levels don't look dramatically elevated.
Common concerns โ muscle aches, liver problems, memory issues โ are more nuanced than many news articles suggest. Serious muscle problems (rhabdomyolysis) are rare. Mild muscle aches affect around 5% of people and often resolve with a dose change or switching to a different statin. If you're concerned about side effects, speak with your GP rather than stopping the medication without advice.
Lifestyle Changes That Genuinely Move the Needle
The evidence base for lifestyle modification in dyslipidaemia is strong. For LDL reduction: replace saturated fats with unsaturated ones (olive oil, avocado, nuts, oily fish), increase soluble fibre (oats, pulses, flaxseed), and add plant sterols if appropriate (found in fortified margarines and yoghurts). For triglycerides: cut refined carbohydrates and alcohol, increase omega-3 rich foods, and lose weight if overweight.
Regular aerobic exercise โ 150 minutes of moderate intensity per week โ is particularly effective at raising HDL, which most medications don't do as well. Even walking counts.
