Blood pressure is the most commonly measured vital sign in medicine โ and yet the majority of people with high blood pressure have no idea they have it. Hypertension causes no symptoms in the vast majority of cases. It earns its nickname "the silent killer" because it silently damages arteries, kidneys, and the heart for years before announcing itself with a heart attack or stroke. Understanding your reading is the first step to protecting yourself.
What Do the Two Numbers Mean?
A blood pressure reading is expressed as two numbers, for example 128/82 mmHg. These are measured in millimetres of mercury (mmHg) โ a legacy unit that remains standard worldwide.
The first number (systolic) โ 128 in the example โ is the pressure in your arteries when the heart beats and pumps blood out. The second number (diastolic) โ 82 in the example โ is the pressure when the heart relaxes between beats. Both matter. Persistently elevated systolic blood pressure is a stronger predictor of cardiovascular events than diastolic in adults over 50.

Blood pressure measured at home is typically 5โ10 mmHg lower than in a clinic โ due to the "white coat effect." The NHS now routinely uses ambulatory blood pressure monitoring (ABPM) โ a device worn for 24 hours โ to confirm hypertension before starting treatment. Home readings are accepted as an alternative: the target at home is 135/85 mmHg, equivalent to a clinic reading of 140/90 mmHg.
Why High Blood Pressure Is Dangerous
High blood pressure is the single largest modifiable risk factor for cardiovascular disease globally. The damage it causes is slow and cumulative โ sustained high pressure stresses the walls of arteries, causing them to thicken, stiffen, and develop fatty plaques. Over years this can lead to: coronary heart disease and heart attack, stroke (haemorrhagic or ischaemic), heart failure (due to the heart working harder against resistance), kidney damage (as the small vessels in the kidneys are particularly vulnerable), and peripheral artery disease.
The risk is continuous and graded โ even in the "high-normal" range (130โ139/85โ89), cardiovascular risk is already elevated compared to optimal. There's no single threshold at which the damage begins; rather, lower is genuinely better.
What Causes Hypertension?
Around 90โ95% of cases of hypertension are classified as "primary" or "essential" hypertension โ meaning no single identifiable cause is found. It results from a complex interaction of genetics, age, lifestyle, weight, diet, and vascular health. The remaining 5โ10% are "secondary" hypertension โ caused by an identifiable underlying condition such as chronic kidney disease, primary hyperaldosteronism, obstructive sleep apnoea, or certain medications (NSAIDs, combined oral contraceptive pill, some decongestants).
Your GP may investigate for secondary causes if hypertension presents in a young person, is difficult to control, or is associated with particular symptoms like headaches, palpitations, or fluid retention.
When Is Medication Recommended?
NICE guidelines recommend starting antihypertensive medication if: sustained clinic blood pressure is 160/100 mmHg or above, OR if blood pressure is 140โ159/90โ99 mmHg (stage 1) AND you have an elevated cardiovascular risk score (QRISK3 โฅ10%), existing cardiovascular disease, diabetes, kidney disease, or evidence of target organ damage. Treatment thresholds are lower for people with diabetes or chronic kidney disease โ typically 130/80 mmHg.
First-line medications depend on age and ethnicity: ACE inhibitors or ARBs are preferred for people under 55 or those with diabetes; calcium channel blockers are preferred for people over 55 or those of Black African or Caribbean heritage. Beta-blockers are less commonly used as first-line antihypertensives but remain useful for specific situations such as heart failure or atrial fibrillation.
Lifestyle Changes That Genuinely Work
Several lifestyle interventions have robust evidence for meaningfully reducing blood pressure โ not as alternatives to medication when medication is needed, but as important adjuncts at every stage:
DASH diet (Dietary Approaches to Stop Hypertension): rich in fruits, vegetables, wholegrains, and low-fat dairy; low in saturated fat and red meat. Studies show reductions of 8โ14 mmHg systolic with full adherence. Sodium reduction: cutting to below 6g salt per day reduces systolic BP by 2โ8 mmHg. The easiest wins are avoiding adding salt at the table and reducing processed food consumption (where 75% of dietary salt comes from). Aerobic exercise: 150+ minutes per week of moderate-intensity activity reduces systolic BP by 4โ9 mmHg. Any sustained aerobic exercise counts โ brisk walking, cycling, swimming. Weight reduction: approximately 1 mmHg systolic reduction per kg of weight lost. Alcohol reduction: drinking above recommended limits (14 units/week) raises blood pressure significantly; cutting down produces rapid measurable improvements. Stopping smoking: smoking doesn't raise resting blood pressure directly but dramatically amplifies cardiovascular risk โ quitting remains the single most important modifiable risk factor reduction anyone with high BP can make.
Checking Your Own Blood Pressure
Home blood pressure monitors are inexpensive, widely available, and highly recommended for anyone who has been diagnosed with hypertension or told their blood pressure was elevated. Use a validated upper-arm device (not a wrist monitor โ these are less accurate). Measure at the same time each day, sitting quietly for 5 minutes first. Take two readings 1โ2 minutes apart and record the average. Share a log with your GP at your next appointment.
โ ๏ธ Seek urgent medical help if your blood pressure reading is above 180/120 mmHg, or if you have high blood pressure alongside: severe headache, vision changes, chest pain, breathlessness, confusion, or weakness on one side. This may represent a hypertensive emergency.
